Reporting adverse events is important

It's really important that staff, students and visitors to our campuses report any adverse events, so that we can put measures in place to prevent reocurrences.

Adverse events are defined as:

  • Accident: an event that results in injury, ill health or damage to the environment;
  • Incident:
    • Near miss: an event that does not cause any harm, but had the potential to cause injury, ill health (including dangerous occurrences) or damage to the environment;
    • Undesired circumstance: 500 Internal Server Error

      The server encountered an internal error and was unable to complete your request. Either the server is overloaded or there is an error in the application.

  • Dangerous occurrence: one of a number of specific, reportable adverse events, as defined in the Reporting of Injuries, Diseases and Dangerous Occurrences Regulations 1995 (RIDDOR)
